This is the Autopsy Report & Death Certificate for Hussayn Abid Mustaf, a detainee at Theater Internment Facility (TIF), Camp Bucca, Umm Qasr, Iraq. It is reported that Mr. Mustaf was using the latrine facilities when, upon reaching for a water bottle, he fell and hit his head. He was transported to the camp hospital for treatment. While at the hospital he began to suffer from his injury, he became unresponsive and required life support measures. Mr. Mustaf subsequently died of his injury. Cause of Death: Closed Head Trauma; Manner of Death: Accident

The Report states that Mr. Karim was admitted to TIF hospital and listed in serious condition with a possible aneurysm. Mr. Karim was then MEDEVAC'd to the Combat Support Hospital (CSH) in Balad, Iraq. The Report states that during the flight Mr. Karim's condition deteriorated and the aircraft stopped in Al Kut. CPR and other life saving measures were initiated, but were unsuccessful. Mr. Karim was pronounced dead. Cause of Death: Hemorrhage of the Right Cerebellum; Manner of Death: Natural.

He sustained shot wounds during the firelight and was transported to the [redacted] for medical treatment. He was later transported to the Central Baghdad Detainee Facility (Abu Ghraib) where he died on 28 April 2004." The document states, "the details surrounding the circumstances at the time of death are classified." The "cause of death" is "asphyxia due to smothering and chest compression." Also found were rib fractures and numerous bruises due to impacts with blunt objects. The "manner of death" is classified as "homicide." The CIA released the first six and last two pages of the report as two separate documents. For purposes of the database, we have combined them. Detainee, Nasef Jasem Ibrahim, died while in U.S. custody at Abu Ghraib prison. Mr. Ibraham, died of natural causes (Cardiac Tamponade as a result of Atherosclerotic Cardiovascular Disease). Mr. Ibraham was brought unconscious to one of the guard stations at Camp Ganci at the prison by his relatives. Once there the MP called for medics found Mr. Abraham was non-responsive and unconscious. They checked for vital signs with negative results and the prisoner was turning blue from the nipples up; pupils were fixed and dilated; no brachial or carotid pulse was detected. CPR was not initiated. The autopsy did not find any signs of trauma, bruising or other wounds.

He was transported to Kadamiya University hospital and was pronounced dead upon arrival. Cause of Death: Heat Related (heat stroke). Manner of Death: Accident. It is reported that Mr. Ibrahim was caught attempting to plant an IED and was being attacked by local Iraqis when American forces came upon the scene and rescued him, and brought him to Camp Cropper for detention & medical treatment. The Report states that Mr. Ibrahim was initially treated for ventricular fibrillation & hypothermia, but upon stabilization he had several complications including a decreased mental status, renal failure, generalized seizures. His conditions were considered irreversible and the decision was made to treat him for comfort. He died seven days later. Detainee, Nasef Jasem Ibrahim, died while in U.S. custody at Abu Ghraib prison. Mr. Ibraham, died of natural causes (Cardiac Tamponade as a result of Atherosclerotic Cardiovascular Disease). Mr. Ibraham was brought unconscious to one of the guard stations at Camp Ganci at the prison by his relatives. Once there the MP called for medics found Mr. Abraham was non-responsive and unconscious. They checked for vital signs with negative results and the prisoner was turning blue from the nipples up; pupils were fixed and dilated; no brachial or carotid pulse was detected. CPR was not initiated. The autopsy did not find any signs of trauma, bruising or other wounds.

He states he was hooded; had hands tied behind his back; placed in a cold small room; sprayed with cold water; and left for two to three days with no food or drink.
