This is the Autopsy Report & Death Certificate of Mukhlef Swedan Sweidan, a detainee who died at the Theater Internment Facility (TIF), Camp Bucca, Iraq. It is reported that Mr. Sweidan underwent throat surgery while in detention. A medical sponge was accidentally left behind and penetrated into the lumen of the trachea. The Report states This sponge led to Mr. Sweidan's death by causing an interference with his breathing. Cause of Death: Complications of Thyroid Surgery (Asphyxia); Manner of Death: Accidental.
