Detainees brought Fawaz Badaa Najem's body to gate at Abu Ghraib during the night after hearing him gasping. He had been otherwise well and had no medical history. The investigation established probable cause to believe that detainee Najem died after being found unresponsive on his cot at night at Abu Ghraib. The cause and manner of death were found to be undetermined after an autopsy was conducted. CID personnel saw no signs of trauma or injury upon initial notification (although autopsy noted small bruises in abdominal area and left arm (p. 14)). Review of medical records found no possible contributors to Najem's death. Najem received emergency care consistent with that of a cardiac arrest. An email included in the report instructs the investigators to "dig up what he was doing, subjected to re: interrogations, etc the days prior to his death" (p. 20). Guards who were interviewed stated that they did not recall any anusual activity by Najem (p. 25). No interviews of camp detainees were conducted except with the initial witness. Investigation closed following determination that "all leads were pursued."

This is an "Operation Iraqi Freedom" investigation.
This investigation was initiated upon notification from the 391 st Military Police (MP) Battalion
(BN), Tactical Operations Center (TOC), BCCF, Abu Ghraib, IZ, that Detainee NAJEM died.
Investigation established probable cause to believe Detainee NAJEM died after being found
unresponsive on his cot at night, while detained at the BCCF. The cause and manner of death
were found to be undetermined after an autopsy was conducted by the Armed Forces Institute of
Pathology. There were no signs of injury or trauma to the body upon examination by USACIDC
personnel at BCCF upon initial notification 14 Jun 04. Interviews of fellow detainees and Camp

On 19 June 04, the undersigned attended the autopsy of F. NAJEM. The autopsy was conducted )11111111111111111111 Armed Forces Medical Examiner, Rockville, MD. During autopsy,
by Dr. (LTC
there were small bruises noted in the abdominal area and left arm, but no other signs of trauma were evident. During the autopsy a small amount of blood was noted in the area of the liver, but this was thought to be post mortem and did not contribute to the death. No obvious cause of death was noted during the autopsy. DrANIMIlindicated she would sign this death out as undetermirled.,

On 21 Jun 04, the undersigned coordinated this investigation with the case agent. The case agent
was instructed to obtain staff duty logs, weather logs and any medical records pertaining to F.
NAJEM.
On 26 Oct 04, the undersigned received the final autopsy report pertaining to F. NAJEM. The report
confirmed the blood in the area of the liver was a post mortem artifact. A review of the report did
not develop any additional investigative leads.
On 1 Nov 04, the undersigned reviewed this investigation to determine if there were any additional
investigative leads to pursue. The review disclosed all leads had been pursued, and the
investigation should be closed.
On 2 Nov 04, the undersigned received an Request for Investigation from the US Army Criminal
Investigation Command, 0581-04-CID001 which alleged an unknown detainee possibly died in June
04 at the BCCF (NFI) as a result of a blood transfusion with the wrong blood type.

NAJEM's death, indicate he received medication and treatment consistent with someone having cardiac arrest. According to the records', NAJEM received no units of blood or blood transfusions.
AGENT'S COMMENT: A crime scene examination was not conducted of the death scene due to
officer safety concerns and operational limitations. The camp contained over 200 agitated
detainees and an insufficient number of Military Police to relocate and guard the detainees,
therefore the death scene examination could not be conducted without placing the agents in harms
way. No canvass interviews were conducted of the camp detainees. The more reliable and less
hostile Camp Chief and initial witness detainee were interviewed. The remaining detainees were
very hostile towards American investigators.
