to the THF OIC: Coalition Provisional Authority Forces Apprehension Form, Two Sworn Statements (DA Forms 2823), Seized Property and Voucher (DA Form 4137).

Responsibility. This is the responsibility of the Capturing Unit Commander/OIC, and should be done when the detainee arrives at the THF (must be completed NLT 24 hours after capture).

48-Hour Report. THF OIC reports the following information to tillOIMMIlbJOC Director:

Date, time, location & general circumstances of capture.
