DA FORM 2823 (Sworn Statement)

Should answer the 6 W's who, what, when, where, why, and witnesses.


Should answer the question, "did [the detained individual] commit a crime?"

If the detainee is a brigade target, state that on the form, along
with information as to why the person is a target. Simply
stating he was picked up because he was a brigade target is
not enough.
• When conducting raids, include in the statement any evidence

Intended to record information and preserve evidence


Yellow fields must be completed. Complete remainder of form as much as possible.

Forms

DA 4137 (Evidence/Property Custody Form)

Records the seizure of evidence associated with a crime.


Captures date-time of seizure, as well as any transfers in custody.


Ensures that transfers are controlled and recorded.


Standard Army Form; should stay with the evidence until signed for at collection point evidence room.

Target information and any information gathered by THTs must accompany detainee packet when detainee is turned in to BCCP

Battalions have 48 hours after capture to get detainees to BCCP


Detainees can stay at BCCP up to 14 days. After 14 days, BG Ham must sign off on keeping detainee at BCCP. Bde conducts a review of packets NLT day 10 IOT determine if detainee should
