CID report in to the death of a detainee in US custody. The detainee was brought to the hospital on December 3, 2003 with blunt trauma to the head with bleeding in the brain (determined by CT scan) and died January 4, 2004. I remember that our front desk had •ome advance notice that this.patient Was .being transported to the hospital. I believe the ira.qi national had a detainee
number but I don't remember that number at this time The person who brought the patient also stated
that he was being interrogated so I. assumed the patient was a detainee: The patient was•-plaCed on a -
stretcher and.rought into the Trauma room, I noticed the patient had a braise approximately 4 centimeters in diameter on his fOrehead. The patient was•uncOnscious andhiS pupils were dilated:: His bloodpressurewas normal, his heart rate was stable ; and he had pending respiratory failure, . The anesthesiologist -• incubated the patient because of his condition, I did not notide•any blood on the patient's clothes and he did
not have any active bleeding, The patient's symptomS were consistent with an anoxic brain injury: . We did not have a CT scan available to us so we did not know what.was the underlying cause was I asked the person who brought the patient in what happened and he stated that he was being interrogated and thathe was sprayed with water on the face and that he thought the patient was a diabetic. The patient was placed ina ventilator and was under observation the entire night, We tried to Mecidevac hiM to the 21 CSH unit that
night bOught there was -some problemi with the weather or there was no available helicopter for the • transport The-patient was finally meddevac'ed in the morning. 1. This memorandum records the details surrounding the death of an Iraqi prisoner in Balad, Iraq
in December, 2003, The patient is said to have been evacuated from the Abu Grath prison
facility and sent to the 21 st Combat Support Hospital (CSH) facility in Balad. This patient had
blunt trauma to the head manifested by severe bruising without lacerations. He also had

on etletainee death in Iraq that was reported to Fort Hood. We did a lot of activity on it and I went ahead and ROI'd it yesterday. SAIIIIIIIMInforrned me she had an RFA out to your office to interview a couple of folks (doctors I think) Your sequence was 204-04 (ours was 0374-04). Anyway,
-we have identified individuals that need to be interviewed or reinterviewPd. I'm unsure if you guys shut down the RFA or what, as we received the info report but the agent working it for us never contacted¦I¦about closing it down. Could be a miscommunication issue, I'm unsure, all I • know I have this ROI and I have some more leads for you guys. Also, could you give me your fax so I can send you some statements; Also, I'm required to report the status of all my CAT I cases every Monday On the attached RFA I have requested your office update me every Friday. It would be a miracle if you can find him, If you have some dead time, no pun intended, and want to try the impossible, I was given this CAT 1 with unknown subject and unknown victim based on the 21st CS11 • commanders recollection when he returned to Hood: He says an Iraqi was brought in with closed head injuries in mid Dec 03, and subsequently died, Said he reported to CID but doesn't think they looked into it because OlD never interviewed his doctors, We were directed to open by higher. The COL also said only 10 to 15 deaths met the criteria from that time frame:

Hope you are doing well in the sand. Made it out of Korea, and to Ft Hood, Tx. Loving it.
This is a tough RFA. Everthing thing needed should be. in Baled at the 31st CSH and/or Mort
Aff. If someone can look at December 2003 SIRs maintained by the 31st CSH, and ensure all
deaths of Iraqi adult males (with closed head injuries) was investigated by CID, we're good to
go. If not, I need to ID who was not investigated. The former COL in charge of the CSH said
only 10 or so fit the criteria .• Let me know if you have any questions about the RFA.
