Camp Roster. The camp roster only referred to Mr. HASSON as being "dead" with no
explanation.

Additionally, based on the PMO Detainee Not in
Camp Roster and the Detainee Personnel

Report, both reflected the victim was captured on 2003/06/04 and processed on 2003/04/09. Assuming the date is written year/month/day, this would indicate the victim was captured on 4 Jun 03 and processed 3 months prior to his capture. If the date is written year/day/month, thiswould indicate the victim was captured on 6 Apr 03 but not processed until 4 Sep 03, 5 months
later. Due to the inadequate record keeping,. this office could only estimate that Mr. HASSON
possibly died between Apr Sep 03.

.1. On 27 Jul 04, this office received information from the
Detainee Assessment Task Force (DATF), in which it was determined

Zubaidy (NFI), died while in

detention at Camp Bucca, IZ.

2. A review of unclassified military intelligence files revealed a

spreadsheet titled "PMO Detainee Not In Camp Roster" which
documented eight detainee deaths, four of which were previously
documented under a CID Report of Investigation (ROI). Mr Al
Zubaidy's death was

one of the four deaths, which was not reported

to CID field'elements for investtatiOn.

Request an investigation be initiated into the above

information.

3. Request acknowledgment of this RFI no later than 3 Aug 04.
Further, request the sequence/report of investigation (ROI) number

RFI, to include sequence/ROI number and of the investigation no later than 10 Aug 04. A response to thisthe status of the

investigation, will be forwarded to this headquarters, ATTN: CIOP-
COP, no later than the above suspense date. All correspondence
will clearly identify the HQUSACIDC control number.

A review of a MS Excel Spreadsheet, titled "PMO Detainee Not In Camp Roster", which was downloaded from the MNC-I C2X website, revealed the death of detainee which was never reported via a CID Report of Investigation. The detainee was identified on the spreadsheet as Hadi Abdul Hussain HASSON AL-ZUBAIDY, National Detainee Reporting System (NDRS) number 300055. A review of the Biometric Automated Toolset Online and other detainee records
revealed no additional aliases.
2. Request that the appropriate CID element determine if this death was a reportable incident

reviewed the patient roster for the 28 th CSH of all Iraqi personnel
roster, it was determine that Mr. HASSON was never treated/or brought to 28 'treated at their facility during the time they (28 th CSH) was operational in Iraq: ErSubsequent to a review of the CSH.
AGENT'S NOTE: The information pertaining to the i ntient roster was received from MAJ11111.111111FPASBA, Fort Sam Houston, TX. M Twas assigned to the 28 th CSH during-hiitinie in Iraq 4744,
The 28th CSH was operational at Dogwood Forwar ogistics Base from 10 April 2003 through 21 August
2003, at which time the hospital transferred operation to Ibn Sina Hospital in Baghdad. The 28

th CSH
transferred operations to the 31 S` CSH on 1 February 2004. Additionally, the 28 CSH established a facility at
Camp Speicher, Tikrit on or about 6 June 2003 through 4 February 2004, at which time operations was
transferred to the 67 th CSH. To date, this office has made numerous attempts to locate documentation which
would provide further details as to the circumstances surrounding Mr. HASSON's death. All efforts have met

with no avail. No more investigative activity is anticipated. at this time. Should additional information be
brought to light at a later time, this office will reopen this investigation.

I. A review of a MS Excel Spreadsheet, titled "PMO Detainee Not In Camp Roster", which was
downloaded from the KINC T C2X website, revealed the death of detainee which was never

reported•via a CID Report of Investigation. The detainee was identified on the spreadsheet as
Hadi Abdul Hussain HASSON AL-Z1JBATDY, National Detainee Reporting System (NDRS)
number 300055. A review of the Biometric Automated Toolset Online and other detainee records
revealed ao additional aliases.

2. Request that the appropriate CID element determine if this death was a reportable incident

This investigation was initiated upon receipt of a Request for Investigation (0373-04-CID001). from the United States Army Criminal Investigation Command. Fort Belvoir. VA, that a review of a spreadsheet titled "PMO Detainee not in camp roster" disclosed Mr. Hadi Abdul Hussain HASSON (Tribal Name: AL-ZUBAIDY), National Detainee Reporting System Number: US9IZ-300055 died while detained at Camp Rucca, Iraq (IZ.)

Preliminary investigation revealed the PMO Detainee riot in camp roster's spreadsheet disclosed Mr. HASSON was detained in Camp Bucca and listed as deceased. It is unknown as to the cause and manner of Mr. HASSON's death.

Request your office coordinate with Detainee Operations. Camp Bucca V. and obtain any and all documentation, to include medical records, screening records, and other related materials pertaining to Mr. HASSON during his internment at Camp Bucca, IZ.

4.
This Ls a Category I Investigation. Results will be provided to this office within 24 hours of receipi. Reasons for non-compliance with the 24-hour response time limit will be expeditiously provided to this office.

Automated Toolset (BATS), Magistrates Cell Database, and In/Out of Camp Roster
with no success. Each number and different versions of the names have been
searched for with negative results. The names are too common to narrow it
down. Any other information, such as date of capture, release date, capture tag,
etc. might help.

We are about 6 Cat 1 RFAs in the hole and 4 Cat 1 ROIs, not to mention 0003-04
(3 man office and I go on leave on Sunday). It might be a few days, but this is a
fairly easy RFA.
