INVESTIGATIVE SUMMARY: About 1815, 9 Aug 03, the 10th MP
BN (CID) reported detainee MOHAMED died while in the
custody of U.S. Forces at the BCCF.

Investigation disclosed that on 8 Aug 03, at Camp Ganci,
Compound 1, BCCF, detainee MOHAMED, while detained by U.S.
Forces, died as result of Arteriosclerotic Cardiovascular
Disease complicated by Diabetes and the manner of death was
natural.

On 24. Aug 03, an autopsy was conducted on Mr. MOHAMED and indicated his cause of death was Atherosclerotic cardiovascular disease complicated by diabetes and his manner of death as natural .

This investigation was initiated based upon notification by the 10 th Military Police (MP) Battalion (CID), Baghdad, 12, which reported Mr. Mohamed died while incarcerated at the Abu Ghurayb Prison, Baghdad, IL.
This investigation established probable cause to believe Mr. Mohamed died of cardiac arrest, sudden death, when he participated in a self-motivated hunger strike while having a history of diabetes.

THIS IS AN "OPERATION IRAQI FREEDOM" INVESTIGATION.

THIS INVESTIGATION WAS INITIATED BASED UPON NOTIFICATION
BY THE 10TH MILITARY POLICE (MP) BATTALION (CID), BAGHDAD,
IZ, WHICH REPORTED MR. MOHAMMED DIED WHILE INCARCERATED AT
THE ABU GHURAYB PRISON, BAGHDAD, IZ.

THIS INVESTIGATION ESTABLISHED PROBABLE CAUSE TO BELIEVE
MR. MOHAMED DIED OF CARDIAC ARREST, SUDDEN DEATH, WHEN HE

PARTICIPATED IN A SELF-MOTIVATED HUNGER STRIKE WHILE HAVING
A HISTORY OF DIABETES.

physician at the prison and provided details of the victims death. MAJallestated Mr.
MOHAMED died from a Myocardial Infarction (heart attack) brought on by his diabetes. For
two days Mr. MOHAMMED was on a hunger strike and did not eat or drink during the time
frame. MAME! attributed the heartache due to the low blood sugar in the victim, which

SSG 54th Quartermaster, BIAP, IZ who provided copies of
the death certificated and a Memorandum For Record, 10 Aug 03. The Memorandum detailed
the summary of events leading to the death of Mr. MOHAMMED. The death certificate listed
the cause of death as Cardiac Arrest, Sudden Death. (See death certificate and Memorandum for
details)

SUMMARY OF MEMORANDUM: At approximately 1945, 8 Aug 03, Mr. MOHAMMED was
complaining of chest pains and had passed out. The patient was a diabetic taking oral 5 mg of
Glibenese twice a day. The patient was participating in a hunger strike for two days consisting
of no food, water or medication. The patient began not feeling well and took a double dose of
Glibenese. Mr. MOHAMMED was treated by medics at the facility in which they proceeded to
administer Cardio Pulmonary Resuscitation (CPR) and administered 2% of Lidocaine. The
victims tongue was swollen; breathing had ceased and was unresponsive to life saving measures.

SFCDand SGT1111111, 414.3' began CPR in an attempt to revive Mr. MOHAMED. Lidocaine 2% 100m1 was injected to stimulate the patients heart and 50% Dextrose was injected to bring up the patients sugar level. After assessing the patient, SGT /111111111111111 noticed Mr.
MOHAMED had no blood pressure due to a weak pulse.
SGT MOM at that time gave the patient an 67r''.3,64 -3
Intravenous catheter . Mr. MOHAMED'S vital signs began 1
to diminish. SGT 111111111111111 made various calls for f,7c3,
assistance over the radio. Eventually SFCalliii6x-3,6‘..-.3
arrived to assist in the situation. Mr. MOHAMED'S
tongue was enlarged and blocking the airway passage.
A J-tube was placed in Mr. MOHAMED'S mouth to maintain
an airway. SGT411111.1111 and SFC ill= injected 67(-3,4-3
Lidocaine 2% 100m1 Intravenously and started Cardio-
Pulmonary Resuiscitation (CPR) in an unsuccessful
attempt to revive the patient.

pronounced dead.

The review of ROI # 0136-03-CID259-61187 determined the investigation was operationally insufficient and was administratively insufficient.

2.
The review disclosed the following deficiencies. The investigation did not conduct a crime scene examination or explain why such an examination was not conducted. The investigation did not conduct any interviews of people with the victim at the time of the incident. A local national medical person mentioned in a narrative summary provided by an U. S. Army medical NCO as having pertinent information was not interviewed. Though U. S. Army medical personnel alleged the victim had a preexisting medical condition aggravated by a self imposed hunger strike, the investigation did not obtain any medical records or conduct interviews to substantiate the information. The case file contained two CD ROM disks and a 3-1/2 floppy disk; however, none of the three disks contained any documents or photographs. The floppy disk was read as unformatted, and the two CD-ROM's contained only a MS Windows operating system "shortcut" to a PDF file. The case file did not contain any copies of the photographs. The original case file did not contain copies of the preliminary or final autopsy reports, or the 1' supplemental report or final supplemental report. The case was opened as an undetermined death, and closed as final (c) accidental death, with the cause of death listed as cardiac arrest. The Investigative Summary block of the Final (c) Report of Investigation contains the caveat for initial reports where the report is a preliminary investigation and may change prior to completion of the investigation.
