This is an "Operation Iraqi Freedom" investigation.
This investigation was initiated upon notification from the 391st Military Police (MP) Battalion
(BN) Operations Section, BCCF, Abu GHRALB, 12, that Detainee FADIL died.
Investigation disclosed Detainee FADIL died as a result of Peritonitis of Undetermined Etiology,
while detained at the BCCF, Iraq.
On 10 Aug 04, this office received the Armed Forces Institute of Pathology Final Autopsy
Report, #ME 04-386, 8 Jul 04, which listed the cause of death as Peritonitis and the manner of
death as natural, which is consistent with previously reported information.

BASIS FOR INVESTIGATION: On 19 May 04, this office was notified by 391st Military Police (MP)
Battalion (BN), Baghdad Correctional Confinement Facility (BCCF), Abu Ghruyeb, Iraq (AGI), that
Detainee Abbas Alwan FADIL, National Detainee Reporting System (NDRS) #160243, BCCF, AGI,
died.
About 1010, 19 May 04, SA -7e---bb -1 and SA' 7c--kA -/ this office, interviewed SSG
76- 3 b4, 7th Combat Support Hospital (CSH), BCCF, AGI, who
stated she was the attending nurse for Detainee FADIL. She stated Fadil was admitted to the
Detainee Ward at 1200, 18 Apr 04, for abdominal pain. She said although he was coughing blood
when he arrived, his pulse was only slightly elevated. She said FADIL stabilized and had normal
vital signs with elevated respiration at midnight. She said between 0620 and 0630, 19 May 04,
FADIL's Nasogastric Tube was removed presumably by Detainee FADIL, which was in place to
relieve pressure from his abdomen. She stated no one saw him remove the tube, but Detainee
:FADIL was awake when they found it removed, which leads her to believe he removed it himself. At
0800, he was transferred from the detainee ward to the emergency room when he became
unstable. Life saving medicines and treatment were given without success. Detainee FADIL was
pronounced dead at 0915 hours by COL (DR.) 348th Forward
Surgical team (FST), BCCF, AGI. (See Death Certificate)
About 1020, 19 May 04, SA 7c -et 1 exposed photographs of Detainee Fadil using a Kodak
Digital Camera. (See Compact Disc)

391st MP BN, Ganci #4 NCOIC, BCCF, AGI, who related that Detainee FADIL had complained
about abdominal pain about 1420, 17 Apr 04, and was transported to the medical hold section for
Treatment. He was later returned to the camp. He said the next morning, about 0930, 20 May 04,
Detainee FADIL was brought to the front gate in a blanket with the same abdominal pain. He was
transported to the CSH for treatment. SFC /C-3 hbl said FADIL never returned to the camp.
About 0945, 20 May 04, SA 7c-eb-1 interviewed Detainee 7C, -3 ofr-.3 -MRS #
bb . 3anci #4 Camp Chief, BCCF, AGI, who stated he was aware that Detainee FADIL was
complaining of stomach pain the day before he was taken to the CSH. He said he was unaware of
any, past medical problem. He also stated that the medics gave Detainee FADIL some medication.
About 0955, 20 May 04, SA 7e interviewed HM2 -le - hb--3 K. Company, 3/24
Marines, BCCF, AGI, who stated he had treated Detainee FADIL on the 17 Apr 04 for abdominal
pain. He said he transported Detainee FADIL to the medical holding area, where he was treated for
dehydration and given anti-nausea medicine.
About 1005, 20 May 04, SA 16-/,6.--/ iterviewed COL 7e.hh.-"who stated Detainee FADIL had a
two day history of abdominal pain, distention, and dehydration. Detainee FADIL was admitted to

the detainee ward, where blood work was drawn and several medications were given for
dehydration pain in his abdomen. He was also given a Nasogastric Tube to relieve pressure from
his abdomen as well as medication to relieve constipation. COLV4%, stated Detainee FADIL was
monitored throughout the night and sometime between 0620 and 0630, 19 May 04, Detainee FADIL
removed his Nasogastric Tube. An attempt was made to place the tube back in without success.
Detainee FADIL was taken to X-ray and while in X-ray, he stopped breathing. He was immediately
brcught to the emergency room where emergency breathing procedures were given. Detainee
FADIL recovered shortly thereafter. At about 0800, an attempt to reinsert the Nasogastric Tube was
made. While inserting the tube, Detainee FADIL stopped breathing again. Cardio Pulmonary
Resuscitation (CPR) was attempted for 20 minutes as well as lifesaving medication without
success. Blood was drawn for testing which revealed Detainee FADIL's Creatine Phospho Kinase
(CPK) was 3400, to which the normal is 100. COL -7d4:5 •tated this indicated massive heart muscle
damage. He said he pronounced Detainee FADIL dead at 0915, 19 May 04. COL-A&3 stated, in
retrospect, he believed in reaction to the dehydration, Detainee FADIL's body cut the blood flow to
non-critical organs of the body, which may have caused the abdominal pain. He also stated,
Detainee FADIL had to have had a history of heart disease due to manner in which he died. (See

About 1400, 22 May 04, SA 7'X161 MP' BN, Detainee Operations NCOIC, BCCF, AGI, who stated detainees have a continuous supply
el potable water available at all times. He also stated that detainees, who do not have access to the
potable water, are provided one liter bottle of water per hour.
out 1400, 23 May 04, SA 7c-/ áb-/: interviewed MAJ (Dr.) 76-3 391st MP BN Aid
Station, BCCF, AGI, who opined that a continuous amount of potable water or one liter of water per
hour is sufficient to prevent dehydration. He also stated, that through his experience with the
detainees, they do not drink the water as they should and are constantly encouraged to drink more.
'On 6 Jun 04, SA 7C '47'1 obtained the Preliminary Autopsy Report, #ME04-387, 1 Jun 04,
pertaining to Detainee FADIL which listed the cause of death as Peritonitis of Undetermined
Etiology and the manner of death as Natural. (See Prelim Autopsy Report)

INITIAL:

THIS IS AN "OPERATION IRAQI FREEDOM" INVESTIGATION.
THIS INVESTIGATION WAS INITIATED UPON NOTIFICATION FROM THE 391ST
MILITARY POLICE (MP) BATTALION (BN). BCCF, OF A DETAINEE DEATH.
PRELIMINARY INVESTIGATION REVEALED AT APPROXIMATELY 1200, 18 MAY 04,
DETAINEE FADIL WAS ADMITTED TO THE DETAINEE WARD FOR ABDOMINAL
PAIN. DETAINEE FADIL WAS COUGHING UP BLOOD AND HIS VITAL SIGNS WERE
SLIGHTLY ELEVATED. DETAINEE FADIL WAS STABILIZED AND UNDER CLOSE
OBSERVATION BY MEDICAL PERSONNEL.

MEASURES WERE PERFORMED, BUT UNSUCCESSFUL, AND AT 0915, 19 MAY 04,
COL (DR) 13 191, 7TH COMBAT SUPPORT HOSPITAL (CSH), BCCF, AGI
(NFI), PRONOUNCED DETAINEE FADIL DECEASED.
A VIEWING OF THE BODY BY USACIDC PERSONNEL REVEALED NO SIGNS OF
EXTRAORDINARY TRAUMA OR INJURY TO DETAINEE FADIL.
AN AUTOPSY ON DETAINEE FADIL IS PENDING.

(MP) Battalion (BN), Baghdad Correctional Confinement Facility (BCCF),
Abu Ghruyeb, Iraq (AGI), that Detainee Abbas Alwan FADIL, National
Detainee Reporting System (NDRS) #160243, died as a result of Acute
Myocardial Infarction (Heart Attack).
