CID investigation into the manner of death of detainee Fathel Ibrahim Mahmood at the Baghdad, Central Confinement Facility, Abu Ghraib, Iraq. Mr. Mahmood experienced chest pains and was discovered unresponsive when medical personnel arrived at his cell. He was pronounced dead soon after. There was no autopsy conducted or investigation at the scene of the incident. Prior to the incident, detainee had complained of pains in his chest and shortness of breath and was given medical attention and medication by Army medics. The CID report acknowledges that there is the possibility of medical negligence, but determines that there were no criminal acts committed. An earlier draft of the report lists cause of death as cardio and pulmonary arrest, but the final report states that the cause of death is undetermined.

INVESTIGATIVE SUMMARY:
This is an "Operation Iraqi Freedom" investigation.
This investigation as initiated upon notification from the Operations Officer, 22nd Military Police (MP) Battalion (BN)(CID), Baghdad International Airport (BIAP) of 'a detainee death at BCF.
Investigation revealed at approximately 0400, 19 Apr 04, an unknown detainee reported to the on duty MP that Mr. MAHMOOD was having chest pains in his cell. Medical personnel arrived at Mr. MAHMOOD's cell and discovered him unresponsive. Life-saving measures were taken, but unsuccessful and at 0435,

Further investigation for additional records revealed the only record available was Mr. MAHMOOD's Detainee Dossier.

Leave it undetermined. If all the leads are complete, close and publish the final supplemental. Did you get a death cert from the doc who declared him? That's something CID CMD is bouncing cases back for. I've aot a list of them that are going to be sent back to the field to do exactly that. You've got two of them, 0013-04 and 0040-04, both of them are from riots where detainees were justifiably shot. You'll need to get DC's ifthey're available, then republish the finals. If not available, publish a final with an AIR documenting the effort to locate them.
Know this is a pain, appreciate the work.

Here is the email traffic on the case in question about the
Undetermined Death.

All the activity has been completed and now I'm wondering what I should
do? Change it back to Natural? Leave it as Undetermined? I figure
your the best expert on this one. This occurred right before "they"
decided we were going to do autopsies.

This case has one interesting fact it occurred on the day when all of
Abu (CID) was getting the new CIF stuff on Camp Victory. I remember
,sked me if I knew anything about this death as

this day because
we were going to get our CIF.

After looking at all the information 1 think the US military did this
guy a great disservice in not treating him properly...then again...I
don't know if the US had the means to treat this guy properly.
Everyone out here in Abu is short-changed in some capacity. But what

No crime scene exam was conducted due to operational safety, due to the number of detainees.

2.
At the time of this incident, detainees were not autopsied, therefore a copy of the report was not provided to the AFIP and an autopsy was not conducted.

3.
At the time of the incident, all CID agents were located on BIAP and did not return until after the body was on the way to BIAP.

4.
Copies could not be obtained of the medical journal at the time of the review, because neither this office nor the medical office had a copy machine. The medical journal could not be collected as evidence, due to the fact that it is a continuous journal with every detainee's medical visits documented. (Copies will be obtained now that we have a copy machine)

5.
The Final Supplemental ROI was a draft, therefore it concentrated on the main body of the case. When it is final it will be correct.

6.
The Camp NCOIC was the MP who reported the death. It will be made clearer in the AIR.

7.
This agent is aware of the medical care issue. However, no criminal act was committed. New procedures have been implemented by the medical staff.

About 1600, 20 May 04, SA 'Xi to&--I interviewed SGT ?CI b6-, 3 391 st MP BN Aid Station, BCCF, AGI, who maintained the medical logs for all detainees. She provided the following information as she referred to the medical logs: On 2 Apr 04, MAHMOOD's blood pressure was checked with a result of 160/94, which she stated was a little high, but not alarming. On 5 Apr 04, he complained of left side chest pain, but had none at the time. He also complained of fatigue. His pulse was 77 with a blood pressure of 134/98. His lungs were ok. He was prescribed Tylenol and Celebrex. On 10 Apr 04, he complained of chest pain and shortness of breath. She could not determine what his treatment was. On 16 Apr 04, he complained of chest pain for the past ten days, but said he had no prior heart condition. He said he did have hypertension; did not have diabetes; and was not experiencing shortness of breathe. He was prescribed Isordil to dilate the arteries, captopril for blood pressure, and aspirin to thin the blood. She stated that was the last entry in the medical journal. She explained the journal was only kept for weekly sick call hours at the camps. At the time of this incident, no journal was being kept for weekends and night medical emergencies. Ibrahim MOHMOOD, NDRS #150214. Detainee 7C..liot,7E-4itated he was a medical doctor for the
Ministry of Health of Iraq. He said about 20 days before the death of MAHMOOD, MAHMOOD
came to him and complained of shortness of breath and chest pain. He examined MAHMOOD and
recommended to the Army Medic that MAHMOOD undergo an Electro Cardio Graph (ECG). He
stated an ECG was never done. Detainee lc, it-who is allowed to prescribe medicine,
prescribed Isorbid (10mg), an artery dilator, for MAHMOOD. He said MAHMMOD got better over the next week or two. He said, on the morning of 18 Apr 04, MAHMOOD was experiencing severe
chest pains. Detainee 1e10)--q 7P referred MAHMOOD to the military medics, but did not feel enough
was done for MAHMOOD. Detainee 117-4 said he was made aware that MAHMOOD was again
having chest pains on the morning of 19 Apr 04. He went to the tent and observed MAHMOOD,
unconscious and beginning to snore. He went for help from the guards and while at the front gate
some unidentified detainees carried MAHMOOD to the gate. ecked for pulse on
MAHMOOD with negative results. A short time later the medics and doctor arrived.
AGENT'S COMMENT: 76-3 U. -3 was fluent in English.
About 1030, 18 May 04, SA lc -1 1111, -1 vith the assistance of Interpreter -7c, bc, this office, interviewed Detainee KA 11-1 fr* 14-NDRS # 7(-116 63nci #1 Chief, who stated he knew MAHMOOD had a heart condition. He related that four days before he died, MAHMOOD fell to the ground with pain and the medics treated him. He was unaware of the results of the treatment.
About 1140, 18 May 04, SA 'terviewed SSG 1 C...11-1 1st Military Police (MP) Battalion (BN), Baghdad Correctional Confinement Facility (BCCF), Abu
Ghruyeb, Iraq (AGI), who stated he was on duty the day MAHMOOD died and reported the death. He stated, referencing a camp log, about 0400, MAHMOOD was brought to the front gate and Shadow Main was notified. About 0415, after 7c0 151,1,-Wdetermined MAHMOOD was dead, Shadow Main was notified that MAHMOOD was dead and lifesaving measures would not be conducted. About 0425, 1LT (DR.) 16' 3 19k2/ 391 st MP BN Aid Station, BCCF, AGI arrived on scene. SSG 7ai /IP/ated the medics treated MAHMOOD on the 18 Apr 04 for heart
burn.
About 1500, 19 May 04, SA 70-4 hb- I 3rviewed MAJ (Dr.) -74-7 3 ) 91 st MP BN Aid Station, BCCF, AGI, who stated he was not aware of any complaints made by Detainee MAHMOOD prior to his death. He stated he became aware of MAHMOOD's medical complaints after his death. DR. *;ko .-.3 Iso stated he was aware of Detainee 7a, k1 7F-(o3pleasure with the medical treatment prior to his death. DR. 7C-t -4erred this agent to the medical logs that are kept by the medics who treat the detainees on a daily basis.

OPERATIONS OFFICER, 22ND MP BN (CID), BIAP, OF A DETAINEE DEATH AT BCCF.
INVESTIGATION REVEALED AT APPROXIMATELY 0400, 19 APR 04, AN UNKNOWN DETAINEE REPORTED TO THE ON DUTY MP THAT DETAINEE MAHMOOD WAS HAVING CHEST PAINS IN HIS CELL. MEDICAL PERSONNEL ARRIVED AT DETAINEE MAHMOOD'S CELL AND DISCOVERED HIM UNRESPONSIVE. LIFE-SAVING MEASURES WERE TAKEN, BUT UNSUCCESSFUL AND AT 0435, 1LT (DR) lc,3r, bip3 91 sT MP BN, BCCF, IZ, PRONOUNCED DETAINEE MAHMOOD DECEASED.

THE BODY WAS TRANSPORTED TO BIAP FOR RELEASE TO THE FAMILY.
