CID report of investigation into the circumstances leading to a detainee entering a vegetative/comatose state. The report states that the detainee was apprehended on July 23, 2003 as a high priority target, and that he was detained in Kirkuk and later in Tikrit, Iraq. On August 11, 2003, he was transferred from the custody of US forces to a combat hospital in a vegetative state. The medical report lists his primary diagnosis as brain damage caused by lack of oxygen. Additionally, the report states he suffered a heart attack and heatstroke. He also had injuries to his head, cuts on his arm and burn marks on the bottom of his feet. The report cites New York Times and Standard News Net news articles about the victim, which report the detainee's name to be Sadiq Zoman and indicate that his injuries were a result of torture experienced in Tikrit. 1. This office received a Request For Investigation from the Headquarters U.S, Army
Criminal Investigation Command (USACIDC) regarding a news article, which alleged Mr.

was tortured while as a detainee at the Tikrit Detention Facility, FOB Danger (formally
FOB Ironhorse), Tikrit, Iraq. He is currently in g coma with extensive brain damage
following his detention at the Tikrit Detention Facility.
2. Preliminary investigation disclosed mriiiiPiesin a vegetative state after leaving
U.S. Forces custody in August 2003 According to U.S Military documentation, on 21 Jul t
03, Mr.11111W„v a s apprehended from his home in 1/11111R, tZ, as a high priority target
due to his affiliation with the Beath Party; Mr. was detained at the Kirkuk.
Detention Facility until 1 Aug 03. wherein; he was transferred to the Central Detention'
Facility, Tikrit IZ. On 11 Aug 03 -,mr..1111111110ttias transferred to the 28th Combat
Support Hospital (CSH), FOB Speicher, Tikrit, IZ, wherein; he was medically treated by
Dr. (LTC) (formally assigned to 28th CSH, FOB
Speicher, Tikrit, IZ): On 21 Aug 03, I14r. was transferred, to the Tikrit local
hospital, Tikrit, IZ, for further medical treatment. LTC medical report listed the 0-c-:(1
primary diagnoses of Mr. onclitiori as hypoxic brain injury (brain damage 1616 -1(
caused by lack of oxygen) "with persistent vegetative state," myocardial infarction (heart
attack), and heat stroke. The same medical report did not mention any bruises, head .
injury, burn marks or other signs of injuries the Iraqi doctorstated they found on Mr,
ody upon his arrival at the Tikrit hospital nearly two weeks later. V•
3. ReporteIndicate, previous care providers had verbally stated, upon transferring Mr.
111/111RO the 28 th CSH that been conscious enough to complain of
"chest -pain that radiated into his arm" earlier that day. At that point, the report states,
Mr Issilipoas treated with a nitroglycerine tablet and intravenous fluids before being
"returned to the prison population," only to be brought back tO medics later, "shaking and
unresponsive."

Iraq concerning the abuse of detained persons by US military and civilian contracted porSonnel, These
investigations have generated significant Department of Defense, Congressional, and international .
attention. Several Congressional and Department of Defense task forces and commissions have been
created to review divergent aspects of detainee operations as well as investigations conducted by the
TJSACIDC. It is expected this scrntfiy and oversight will continue for the foreseeable firture:
3, Effective 25 May 04, the Commanding General, USACTDC, has designated all detainee abuse
ROIs as category I rnonitorship. All field elements are directed to reference lb for timeliness and
timely reporting requirements_ In addition, all ROIs dispatched effective this date will be SSI reportable

We were directed to open by higher. The COL also said only 10 to 15 deaths
met the criteria from that time frame.

Got it Here is our last update; Just spoke with the MP CDR for the one of
detention facilities during your time frame, He had an op con platoon (978th
MP Co, Fort Bliss , TX) who was Manning the facility at the time If your guy
was picked up by the 173rd, he went to his detention facility, They kept a lot of
records ie intake and release to medical facilities etc MP CDR thinks most of
the infO is still in country.
If he was picked up by TF guys or 1/22 he went to the Div Temp Detention
Facility, also there. The only reason I know this is because I am currently
working another CAT 1 where a 1/22 detainee "disappeared" for 19 days, and
ended uP.L?eafen. Good luck. . .. •

have interviewed SOT and SSG who was the NCOIC of the
detention facility during the 9713th's rotations. Neither had any
information pertaining to knowing-who Mr. was, howeVer, they both 6q-C -qt66 -4(
remembered a detainee that was brought•to the detention center unconscious
in the back of the truck. Neither could identify the delivering unit, but
they both suspected thg truck and it's detainees came from'F03 Warhorse. I
saved their statements to fire if You•need them;..
sqrsimiwi. currently on leave -in the Denver, CO' area-, X don't have
leave address, will get that tomorrow (Wednesday) from the S-1. I do have
his - cell phone number, will provide that along with the RFA to the Carson
I also have SGT ETS address, it's here in El Paso, TX. I'm about '
to call the residence and determine if he's in the area.- That.rs all for
