1. You are hereby appointed an investigating officer pursuant to R 15-6 to conduct an

informal investi ation into the allegations of misconduct against Commander currently at FOB Buzz and is suspended from command until further

In your investigation, use formai procedures under AR 15-6. You will make findings as to the catise..oflhe accident and recommendations for preventing future occurrences.

If in the course of your investigation you come to suspect that certain people may have acted inappropriately, you must advise them of their rights under the UCMJ, Article 31, or the Fifth Amendment, as appropriate. In addition you must provide them a Privacy Act statement before-you solicit any personal information.
4.
Submit your findings and recommendations to me within 72 hours or notify me if an extension is needed.
